is my lender bank of america or great lakes?

I have a stafford student loan. great lakes sent me the master promissory note for it which I lost so they told me to fill one out on bank of america website. I want to pay it back who should I be contacting BOA or great lakes and who can I ask for a receipt or something to prove that it has been paid back?
